HOW DOES OC3 USE HMIS?

WellSky Community Services is a Management Information System used to record and share information among service-providers on services provided to at-risk, homeless, or near homeless clients. WellSky Community Services is the Homeless Management Information Services (HMIS) vendor for OR-505 Oregon Balance of State Continuum of Care (CoC). OC3 is one of several providers who utilize WellSky for HMIS in Oregon. WellSky was previously named ServicePoint. 

There are tons of benefits for utilizing HMIS:

  • Track client progress and outcomes
  • Coordinate services to provide appropriate and timely care
  • Information for community needs and improve service approach

Individuals and families experiencing homelessness...

  • Decreased duplicative intakes and assessments
  • More streamlined resource navigation
  • Can easily request simple updates to situation with digital records

OR-505 has over 20 Agencies with more than 100 Projects in WellSky as of January 2026.

HMIS tracks projects that are any of the following:

  • Emergency Shelter
  • Day Shelter
  • Homeless Prevention
  • Transitional Housing
  • Permanent Housing (no disability required for entry)
  • Permanent Housing (Rapid Re-Housing)
  • Permanent Supportive Housing (disability required for entry)
  • Street Outreach
  • Services Only
  • Haven
  • Other
